Title: Hypertension Risk from Iron Brake Particulate Matter

William J. Rowe M.D. FBIS (Fellow British Interplanetary Society), FACN (Fellow American College of Nutrition, Retired Fellow Royal Society of Medicine), is a board certified specialist in Internal Medicine. He received his M.D. at the University of Cincinnati and was in private practice in Toledo, Ohio for 34 years. During that time he supervised over 5000 symptom - limited maximum hospital-based treadmill stress tests. He studied 3 world class extraordinary endurance athletes and published their exercise-related magnesium deficiencies. This triggered a 20 year pursuit of the cardiovascular complications of Space flight. 

Abstract:

Of 12 moon walkers, James Irwin on day after return from Apollo 15 mission, showed extraordinary bicycle ( B) stress test (ST) hypertension ( 275/125) after 3 minutes exercise; supervising > 5000  maximum treadmill  ST, author never witnessed ST- blood pressure approaching this level.  Symptom-limited maximum B stress test showed “cyanotic fingernails”; possibly venous blood trapped peripherally, supporting author’s “Apollo 15 Space Syndrome,” postulating that severe fingertip pain during space walks, triggered by plasma fluid, trapped distally;  mechanism could be related to endothelial dysfunction, providing “silent ischemia” warning.  Neil Armstrong returned to Earth with severe diastolic hypertension  ( 160/135), consistent with ischemic left ventricular dysfunction; 50 mm increase in comparison with resting  BP 110/85. With inhalation of lunar dust, brought into habitat on space suit, with high lunar iron (I) this dust inhalation, along with reduced (R) space flight- transferrin, R antioxidant, calcium  (Ca) blocker -  magnesium, conducive to severe oxidative stress, Ca overload  with potential endothelial injuries. Using moon walker studies as example, my recent editorials show that I dust, released from brakes, with over 90% of brakes made of I, is a major hypertension factor and may  also contribute to myocardial infarctions.

Mauceri Filippo graduated in nursing in 2018 with a grade of 103 out of 110. He worked for 4 months in medicine and now works in a nursing home. On 4 April 2019 he presented his study on sepsis at the National Congress of Riccione and this study was awarded as the first research of all congress

Abstract:

Sepsis is a very frequent and serious disease, although still poorly understood. It affects 20-30 million people worldwide, about 250,000 in Italy alone, of which 1 in 4 does not survive. The purpose of my research is to understand how sepsis is managed in the Emergency Room, what is still to be improved and what can be improved. The study population includes patients admitted to the Emergency Unit of Siena from 01/09/2017 to 02/28/2018, who respond positively to inclusion in the study. At the end of the data collection the sample includes 102 patients. Many variables have been studied that help to understand nursing management of septic patients. The variables: Sex; Age; Methods of arrival at the emergency room; Admission codes used; Discharge codes used; Patients who received the qSOFA score; Values qSOFA; Patients who were given the antibiotic within the first hour; Patients who have been treated with Procalcitonin; Patients who underwent blood gas analysis; Average PCR, Procalcitonin and Lactate values; Patients whose blood cultures were performed; Results of blood cultures performed; Patients who were treated with other cultures; Etiology of bacteria resulting from blood cultures; Etiology microorganisms resulting from other cultures; Patient hospitalization time; Frequency of septic patients at the emergency room in the various months; Follow up. These variables collected if analyzed in multiple hospitals could make a difference in the management of sepsis with the possibility of saving more lives.

Symptom Definition is the only parameters  necessary for a Symptom. As any or all other definitions, symptom definition should be describe the symptom scientifically. If it cannot describe clearly, there is no use of  a symptom  definition. A symptom is a departure from normal function or feeling which is noticed only by a patient, indicating the presence of disease or abnormality.

Jungmin Lee has completed her PhD from College of Nursing, The Catholic University of Korea. She is RN, Department of Nursing, Samsung Medical Center. She has worked in the neurosurgery intensive care unit for 13 years and is very interested in research. She had a number of research and book publishing and development of practical guidelines.

 

Abstract:

The goal of this study is to apply by a useful algorithm in clinical environments, verifying the effectiveness of the Evidence based guideline for fever management of critical adult patients with brain injury. Methods: This nonequivalent control group pretest-posttest experiments involved 48 patients aged ≥ 19 years who were admitted intensive care unit after surgery due to brain injury. Authors investigated the rate of patients with reduced fever, time to decrease fever the difference of neurological status when they discharge the ICU, between either with the algorithm for fever management or not was analyzed. Results: The rate of the patients whose fever was decreased after the first fever was the rate of the patients with controlled fever in experimental group is 4.50 times higher than the control group(95% CI: 1.22-16.60, ρ = .024). The fever of 50% of patients in experimental group reduced within less than 1 hour, while in control group, it took less than 17hours (Log rank ρ < .001), the experimental group takes 4.8 times quicker to decrease fever than control group(HR: 4.84, 95% CI: 1.79-13.11, ρ = .002). Neurological status when to admit and to discharge was1.25±2.58 points in experimental group, and 2.13±2.70 points in control group, which was no significant difference between the two groups (z=-1.43, p = .253).Conclusion: Algorithm for fever management of brain injury patients can be a scientific evidence for nursing practice with fast decisions, improving the efficiency of health care, hence contributing to better outcomes for critically ill patients.

Nurses play an important role in the disease prevention, the health promotion, and delivering primary and community care. They will be a key to the achievement of universal health coverage. Based on WHO reports there is a global shortage in the health workers especially the nurses and midwives, who represent more than 50% of the current shortage in health workers. The shortage in the nursing field has an effective role not only on quality of care but also on population health outcomes. WHO estimates that the world will need an additional 9 million nurses and midwives by the year 2030.

Abstract:

Stress is an inevitable part of life for both nurse practitioner faculty and students that can be a motivator or a deterrent to success and productivity. Eustress creates an impetus to move forward in our mental capacity for gaining knowledge or in our physical ability to engage in active learning or to escape danger. Distress also impacts the mind and body on many levels, often leading to inflammation, chronic diseases, anxiety and depression. Yet, current research on mindfulness practices and relaxation indicates that stress can be managed in a way that reduces the negative physical and mental impacts of stress. By implementing simple stress reduction techniques into the classroom faculty can encourage a positive learning environment. In addition, through regular practice of relaxation techniques both students will be familiar with mind-body therapies that may be useful in their personal lives and in clinical practice settings with patients.

Abstract:

Pakistani women are using abortion as part of their strategy to avoid unwanted or mistimed births, notwithstanding the illegality of the procedure and the considerable health risks it entails, as evidenced by the large number hospitalized for treatment of complications each year. The need to seek recourse to abortion is likely to be especially prevalent among women who fear that contraceptives will damage their health, who believe that their husbands object to family planning, or who feel that religious and social norms do not endorse contraceptive use. In addition, many women may have difficulty obtaining the modern methods they need. Under current circumstances, many Pakistani women are paying with their health—and even their lives—to avoid births that they cannot afford or do not want. Helping them avert unintended pregnancy and supporting them in achieving their fertility goals would significantly reduce maternal morbidity and mortality and the associated costs to families, communities and society as a whole.

Parenteral Nutrition "PN" is considered to be complicated and expensive treatment and can cause various complications which demands collaboration between the skilled staff members in order to give the appropriate PN dosing rate, and dosing time which is required  for each patient with through out, following up possible complications and side effects.  In carmel medical center, infusion pack is delivered by a pharmacist according to prescription given from PN staff, and afterwards the Infusion instructions are recorded by one of the department physicians in the patient EHR. Recently there have been several mistakes that have been reported, which made it urgent to check matching between PN staff decision and the record of instructions in EHR.

Abstract:

Errors in medical processes are not entirely avoidable. When these mistakes occur, they are involuntary and unpredictable, so medical staff and especially nurses during therapy should minimize the potential risks (Wolf, 1989). To avoid mistakes and evaluate the work of nurses, they were observed in order to increase the performance of health services (Verhonick, Nichols, Glor, McCarthy, 1968). The purpose of this research is to evaluate the work and ethics of the nurse in relation to the patient in primary care in Kosovo.

In this research, 74 nurses were observed during several days in primary care in Kosovo. To achieve this goal, nurses were observed using the English model Pay for Performance (P4P) model that was tailored and tested according to the nursing processes performed in primary care in Kosovo. During the assessment a test was created which was completed by the observer, and contained 30 nursing processes divided into 4 sections; kindness, management of therapy, hygiene and administrative affairs.

The health system in Kosovo is a flow system designed to be managed at 3 levels of health care. Management systems are divided into primary, secondary and terciary (Kosovo Assembly, 2004). According to KDI (2011) functional health insurance systems are a must to enhance the quality of health services. Since 2011 it has been tried to create drafts to implement health insurance but still without success (FES, 2018). The purpose of this research is to assess the need for health insurance in Mitrovica and also to assess the satisfaction of citizens of this municipality with health services and system.

Abstract:

MERS-COV is a viral respiratory disease caused by coronavirus, that was identified first in KSA in 2012. Infection is the second most common cause of death in dialysis patients, and this is because they were having routine trice weekly hemodialysis and exposed quickly to pathogens through needling, transfusion, and dialysis catheters. A treatment room is ideal for infection transmission. Dialysis patients were prone to severe infection because decreased immune function. Coronaviruses could cause highly contagious pneumonic infections.Spread mainly through contact and respiratory droplet
